World Championship leader Chris Holder and his Elite League table-toppers Poole have confirmed they will face Lakeside in the play-off semi-finals.
The first leg takes place at the Arena-Essex Raceway on Monday night with the return at Wimborne Road on October 1.
Swindon visit Birmingham in the other semi-final, first leg this Monday, before welcoming the Brummies to Blunsdon a week later.
The Robins gained a big psychological advantage ahead of this meeting with a 46-44 triumph at Perry Barr, courtesy of 15 paid 16 points from Troy Batchelor and 13 paid 15 from Peter Kildemand.
Peterborough were unable to overhaul Birmingham or the Hammers for a top-four spot after their heavily depleted side went down 52-40 at Coventry in their final league match.
